If your school has concerns (i.e. academic, behavior, communication, etc.) for a specific student, please complete this form to notify the Office of Special Education. Please note: If the Office of Special Education has not previously been involved with a case, OSE Consult should be requested first prior to an LRE Consideration request. You will receive confirmation of your submitted referral from a Supervisor of Special Education within 24 hours.
